Cobb Churches & Ministries Needs and Assets Survey

Dear Fellow Servants,

Thank you for participating in our survey. Your feedback will assist the Cobb Faith Partnership, along with the Cobb Community Collaborative, in ongoing efforts to equip area churches and ministries as well as establish linkages among the faith community and social service organizations in providing services and outreach ministry tailored to our community’s specific needs.

This survey will take approx. 5-8 minutes of your time. Please answer all applicable questions to the best of your ability, being as specific as you can (The more specific you are with your responses, the more useful the info will be for your program as well as for community planning purposes). Feel free to leave blank those questions not applicable to your situation. All answers will be kept confidential; responses will only be used in aggregate to identify and map where existing services and outreach are located, as well as to create long-range plans to address gaps in existing services and outreach as appropriate.

NOTE: If you wish to receive a copy of the final results of this survey when tabulated, Please be sure to complete the bottom portion of this form. Please also remember to complete the fill-in blanks specifying your "City" and "Zip Code" in Section 15 below, as results will be segregated by location.
